The Health and Wellness Navigator will play a key role in connecting guests and residents to vital health and wellness resources, helping individuals access care, reduce disparities, and build healthier lives.
Role: The Health and Wellness Navigator (HWN) is a professional position dedicated to improving the health and well-being of individuals and families served by Back Bay Mission. This role provides comprehensive support to guests of the Micah Day Center, Food Pantry patrons, Housing Rehabilitation recipients, and the broader Mississippi Gulf Coast community.
The HWN will serve as a critical connector between those in need and appropriate health and wellness resources, working to reduce health disparities and promote equitable access to care. This includes direct engagement with guests, collaboration with internal staff, outreach to the community, and coordination with external service providers.
Salary Range: $35,000 to 40,000 depending on experience, plus fully covered health insurance and a generous pension.
Candidate Profile: Health and Wellness Navigator (HWN)
The Health and Wellness Navigator (HWN) will play a vital role in connecting guests of Back Bay Mission to appropriate health and wellness services across the community. Based in our Micah Day Center, the HWN will engage directly with guests during most service days, building trusting relationships, and identifying individual needs.
Key Responsibilities:
Collaborate closely with Micah Day Center staff and Housing Case Managers to ensure holistic support for all guests.
Provide targeted health and wellness services to residents of Homeport I and II—supportive housing for formerly unsheltered veterans.
Extend health and wellness support to individuals and families served through the Home Rehabilitation Program.
Educate community members—both individually and in groups—about health inequities and disparities through seminars, health fairs, outreach meetings, and media campaigns.
Refer clients to a broad network of community resources and partner agencies; conduct follow-ups to ensure referrals are effective and clients receive appropriate care and support.
The ideal candidate will be passionate about community health, adept at building rapport with diverse populations, and committed to addressing health disparities through direct service and advocacy.
